How IRMAA Works
IRMAA is based on your modified adjusted gross income โ your adjusted gross income plus any tax-exempt interest. Because of the two-year lookback, a big one-time income event (selling a home, a Roth conversion, capital gains) can push you into IRMAA even if your current income is lower.

Higher Income Means Higher Premiums
If your income is above the first threshold, a surcharge is added to both your Part B premium and your Part D premium.
