Higher-income Medicare beneficiaries pay more for Part B and Part D through IRMAA โ€” an income-related surcharge. Enter your income and filing status below to estimate your 2026 surcharge.

How IRMAA Works

IRMAA is based on your modified adjusted gross income โ€” your adjusted gross income plus any tax-exempt interest. Because of the two-year lookback, a big one-time income event (selling a home, a Roth conversion, capital gains) can push you into IRMAA even if your current income is lower.

Social Security Looks Back Two Years

Social Security looks back at your tax return from two years ago to decide whether you owe IRMAA today.

Higher Income Means Higher Premiums

If your income is above the first threshold, a surcharge is added to both your Part B premium and your Part D premium.

Life-Changing Event?

Retirement, marriage, divorce, or the death of a spouse may reduce your IRMAA if your income has dropped.

You Can Appeal

Frequently Asked Questions

What is IRMAA?
IRMAA (Income-Related Monthly Adjustment Amount) is an extra charge added to your Medicare Part B and Part D premiums when your income is above a set threshold. It is not a separate bill โ€” it is added to your premiums.
What income does 2026 IRMAA use?
Your 2026 IRMAA is based on the Modified Adjusted Gross Income (MAGI) from your 2024 tax return โ€” a two-year lookback. If your income has dropped since then, you may be able to appeal.
How do I appeal or lower my IRMAA?
If a life-changing event (retirement, marriage, divorce, or the death of a spouse) lowered your income, file Form SSA-44 with Social Security to request a reduction. Call us and we can walk you through it.
